Nurse Practitioner – Nephrology NP Summary

Duties & Responsibilities
- Conduct comprehensive health assessments, physical exams, and risk evaluations for nephrology patients
- Order, interpret, and evaluate diagnostic tests to inform and tailor patient care plans
- Develop and document individualized treatment strategies in collaboration with physicians and other providers
- Prescribe medications, therapies, and interventions in accordance with current evidence-based guidelines
- Monitor patient progress, reassess and modify treatment plans to optimize health outcomes
- Educate patients and families on disease management, preventive strategies, and lifestyle modifications
- Participate actively in care coordination with internal teams to ensure seamless, high-quality patient care

Qualifications & Requirements
- Master’s degree in Nursing (MSN) required
- Minimum of 2 years’ experience as a Nurse Practitioner, preferably with nephrology or internal medicine
- Current, unrestricted RN and NP licenses in California
- Board Certification as a Nurse Practitioner
- Demonstrated experience managing patients with chronic conditions is strongly preferred
- Bilingual English/Spanish skills are a plus
