Nurse Practitioner Manager (NP Manager) - Summary
- Full-time leadership and clinical position managing a team of Nurse Practitioners in mobile and home-based primary care

Duties & Responsibilities
- Deliver in-home primary care to patients across Los Angeles County, managing acute and chronic conditions
- Conduct comprehensive assessments, medication management, and patient/family education to promote optimal health outcomes
- Lead urgent, same-day follow-ups and conduct various types of visits including post-discharge, wellness, and routine check-ins
- Oversee clinical operations of the mobile primary care team, supervising Nurse Practitioners in the field
- Review and triage new patient referrals, ensuring appropriate care placement and caseload distribution
- Monitor and analyze key performance metrics such as referral volumes, retention, and hospital readmission rates
- Conduct monthly performance reviews and foster a culture of clinical excellence, collaboration, and operational efficiency
- Participate in interdisciplinary team huddles, daily clinical rounding, and care coordination meetings
- Collaborate with community resources, home health agencies, and referring clinicians to assure seamless care transitions
- Promote patient and caregiver education to support adherence and self-management
- Lead advance care planning and goals-of-care discussions with patients and families
- Support hospice eligibility assessments and transitions as appropriate

Qualifications & Requirements
- Master of Science in Nursing (MSN) required
- Active, unrestricted RN and Nurse Practitioner licenses
- Minimum of five years of clinical experience as a Nurse Practitioner (NP)
- Prior leadership or supervisory experience strongly preferred
- Background in home-based care, geriatrics, palliative care, or complex chronic disease management is a plus
- Excellent clinical judgment, communication, and team leadership skills
- Ability to function independently in a mobile care environment
