Nurse Practitioner Manager (NP Manager) - Summary

Duties & Responsibilities
- Provide in-home primary care to patients across Los Angeles County, addressing a range of medical complexities and chronic conditions
- Deliver direct clinical services including assessments, medication management, treatment adjustments, and patient/family education
- Perform urgent and same-day follow-ups based on changes in patient condition
- Manage chronic illnesses with a focus on stability and preventing unnecessary hospital or emergency room visits
- Identify and address care gaps such as preventive screenings, immunizations, medication reconciliation, and safety assessments
- Conduct various visit types, including post-hospital discharge, post-SNF transition, annual wellness visits, routine follow-ups, and care for homebound patients
- Participate in daily clinical rounds with supervising physicians to review patient cases and align care plans
- Maintain a panel of homebound and palliative care patients, including leading goals-of-care and advance care planning discussions
- Evaluate hospice eligibility and facilitate transitions to hospice when appropriate
- Supervise and support the Nurse Practitioner team, providing guidance and mentorship
- Review and triage patient referrals to ensure proper care placement and caseload balance
- Analyze team performance metrics such as referral volume, case retention, hospital readmissions, and ER diversion
- Conduct monthly performance reviews and identify areas for improvement
- Promote a culture of clinical excellence, operational efficiency, and interdisciplinary collaboration
- Coordinate with community resources, home health agencies, DME vendors, and other partners for seamless patient care
- Ensure effective communication with referring clinicians and clinic-based teams
- Support patient and caregiver education to enhance self-management and adherence

Qualifications & Requirements
- Master of Science in Nursing (MSN) required
- Current, unrestricted RN and Nurse Practitioner license
- At least 5 years of clinical experience as a Nurse Practitioner
- Prior experience with team leadership or supervision strongly preferred
- Background in home-based care, geriatrics, palliative care, or chronic disease management is an asset
- Strong clinical judgment, communication, and leadership skills
- Ability to work independently in a mobile care environment
