Nurse Practitioner – Geriatric Medicine (NP, APRN) Summary
- Deliver specialized care to older adults in a clinical setting
- Work collaboratively with physicians, nursing staff, and support personnel
- Promote patient well-being and autonomy through holistic, evidence-based interventions

Duties & Responsibilities
- Perform comprehensive assessments, including medical histories, physical examinations, and cognitive evaluations for geriatric patients
- Develop and implement individualized care plans in collaboration with multidisciplinary teams
- Order, interpret, and evaluate diagnostic tests to inform clinical decisions
- Prescribe medications, therapies, and other interventions within the scope of practice and according to state licensure regulations
- Educate patients and families on disease processes, treatment options, and preventive care strategies specific to geriatric health
- Monitor patient progress, modify care plans as needed, and document all care in accordance with legal and organizational policies
- Facilitate transitions of care, including discharge planning and coordination with community resources
- Participate in quality improvement initiatives and contribute to program development in geriatric medicine

Qualifications & Requirements
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
- Active and unrestricted California state license as a Nurse Practitioner
- Nurse Practitioner Furnishing (NPF) license number
- Current NP board certification in Adult Acute Care or Adult Gerontological Acute Care (required for inpatient clinical care)
- Active DEA certificate to prescribe controlled substances (II-V)
- Active Basic Life Support (BLS) certification from the American Heart Association (AHA)
- Active Advanced Cardiovascular Life Support (ACLS) from AHA
- Relevant experience as a Nurse Practitioner in acute or ambulatory geriatric care settings
