Medical Director, Primary Care (MD/DO) - Summary- Leadership role overseeing a primary care outpatient center for seniors
- Focus on value-based, patient-centered care and improved health outcomes

- Lead and manage primary care physicians and nurse practitioners within the center
- Oversee a small patient panel (up to 225 patients) focused on senior care
- Implement and monitor value-based care initiatives for seniors with multiple chronic diseases
- Participate in direct patient care and clinical decision-making
- Ensure high standards of quality, safety, and patient satisfaction
- Collaborate with onsite specialists and holistic health practitioners
- Engage in ongoing leadership and operational training programs

- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- At least one year of clinical experience in a geriatric or family practice setting
- Demonstrated leadership skills; prior clinical leadership experience preferred
- Strong communication, analytical, and organizational abilities
- Proficiency in Microsoft Office suite
- Ability to travel locally and occasionally regionally as needed
