As the Medical Director, Primary Care (MD/DO), you will oversee a multi-disciplinary team of primary care providers, including physicians and nurse practitioners, working collaboratively to deliver personalized, respectful, and accessible care to a senior patient population. The position emphasizes value-based care, focusing on managing chronic diseases and improving the overall health outcomes for seniors. Your expertise will guide the clinical direction, foster a culture of continuous improvement, and drive operational excellence throughout the center.

This role offers a balanced Monday through Friday work schedule, promoting work-life integration and professional satisfaction. In this dynamic setting, you will have direct patient care responsibilities, guiding complex care management for seniors typically managing three to five chronic conditions. In addition, you will mentor and develop a team of primary care providers, ensuring adherence to best practices and fostering a patient-centered environment.

Candidates should have at least one year of clinical experience in a primary care or geriatric setting, with prior leadership experience strongly preferred. Strong communication, leadership, and analytical skills are essential, as well as proficiency in Microsoft Office and the ability to travel locally and occasionally regionally as needed.
