This role is designed for a board-certified or board-eligible physician in Internal Medicine, Family Medicine, or Geriatric Medicine who is enthusiastic about value-based care models and senior patient populations. The Medical Director, Primary Care – MD/DO, will manage a small patient panel of seniors with multiple chronic conditions and provide leadership to a team of primary care physicians (PCPs) and nurse practitioners (NPs). Medical Director, Primary Care – MD/DO Summary
- Lead a value-based primary care clinic focused on senior patients
- Guide clinical operations, mentor providers, and ensure delivery of high-quality care
- Participate in organizational leadership training and development
- Deliver care to a small, manageable patient panel for optimal engagement and outcomes
- Collaborate with in-house specialists and holistic health service providers
Duties & Responsibilities
- Provide clinical leadership and oversight for a primary care clinic serving seniors
- Supervise and mentor primary care physicians and nurse practitioners
- Direct and manage care for a patient panel of 60–90 seniors with complex, chronic conditions
- Implement and monitor value-based care initiatives to improve patient outcomes
- Participate in leadership development programs, including Medical Director and Center Operations, Medical Economics, and Center Financial Performance
- Engage in ongoing professional training, such as PCP Black Belt Training and PCP Essentials
- Foster a positive, collaborative clinic culture and support organizational growth strategies
- Work closely with in-house consultants in specialties like cardiology and podiatry, and with holistic service teams offering acupuncture, Tai Chi, yoga, and nutritional guidance
- Support clinic operations including patient scheduling, quality improvement, and policy development
- Promote the highest standards of patient safety, satisfaction, and evidence-based care

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- At least 1 year of clinical experience in a geriatric or family medicine setting
- Previous leadership experience preferred
- Strong leadership, communication, and analytical skills
- Proficiency in Microsoft Office
- Ability to travel locally and occasionally regionally as needed
