Medical Director, Primary Care (MD/DO) - Summary- Lead and manage an interdisciplinary clinical team focused on primary care for seniors.
- Champion value-based care and implement innovative care models for chronic disease management.
- Participate in strategic planning and ensure compliance with organizational and regulatory standards.
- Engage in ongoing training and professional development in leadership, operations, and medical economics.
- Oversee and mentor primary care physicians (PCPs) and nurse practitioners (NPs).
- Manage a patient panel of 60-90 seniors, ensuring personalized and comprehensive care.
- Monitor and improve clinical performance metrics, including patient outcomes and satisfaction.
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) for integrated patient care.
- Promote holistic wellness through access to services like acupuncture, tai chi, yoga, and nutrition counseling.
- Ensure seamless coordination of patient transportation and other support services.
- Participate in leadership training, operational planning, and growth initiatives.

- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or related specialty.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Minimum of 1 year clinical experience in geriatric or family practice setting.
- Leadership experience preferred.
- Strong leadership, communication, and analytical skills.
- Proficiency in Microsoft Office Suite.
