Medical Director – Primary Care (MD/DO) – Summary
- Lead clinical and operational excellence in a primary care outpatient setting.
- Oversee a small patient panel of seniors with multiple chronic conditions.
- Champion value-based, holistic care delivery and interdisciplinary collaboration.
- Serve as a mentor and leader to Primary Care Physicians (PCPs) and Nurse Practitioners (NPs).
- Drive continuous improvement through leadership training and operational best practices.
Duties & Responsibilities
- Supervise clinical care for a maximum of 60-90 patients, focusing on seniors with 3-5 chronic diseases.
- Manage and mentor a team of PCPs and NPs, fostering a collaborative and supportive work environment.
- Implement and promote value-based care strategies to achieve optimal patient outcomes.
- Participate in and complete leadership development programs, including PCP Black Belt Training, Medical Director Essentials, and Leadership Pathways.
- Monitor and optimize center financial performance, patient satisfaction, and clinical quality metrics.
- Coordinate with in-house consulting specialists such as cardiology, podiatry, and more to provide comprehensive care.
- Promote holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure patient access to care, including facilitating door-to-door transportation for eligible patients.
- Maintain compliance with regulatory and organizational standards.

Qualifications & Requirements
- Board Certified/Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- MD or DO degree from an accredited institution.
- At least 1 year of clinical experience in a geriatric or family practice setting required.
- Leadership experience in a clinical environment preferred.
- Excellent leadership, communication, and analytical skills.
- Proficient in Microsoft Office Suite.
- Ability to travel locally and occasionally regionally as needed.
