Medical Director – Primary Care Summary
- Lead clinical operations in a primary care outpatient setting focused on seniors with complex chronic conditions.
- Foster a patient-centered care model with a reduced patient panel for enhanced patient relationships and outcomes.

Duties & Responsibilities
- Direct daily clinical operations and provide medical oversight for the outpatient primary care team.
- Manage a reduced patient panel (60-90 patients) with a focus on seniors experiencing multiple chronic diseases.
- Facilitate value-based care initiatives, ensuring optimal quality and cost outcomes for patients and the healthcare system.
- Mentor and supervise primary care physicians (PCPs) and nurse practitioners (NPs), providing ongoing feedback and professional development.
- Participate in leadership training, financial performance management, and operational pathway programs.
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other specialties to deliver coordinated patient care.
- Support holistic healthcare services such as acupuncture, nutrition counseling, and wellness activities.
- Engage in community outreach and support organizational growth initiatives.

Qualifications & Requirements
- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board certified or board eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Minimum of 1 year clinical experience in a geriatric or family practice setting; leadership experience preferred.
- Demonstrated leadership, communication, and analytical skills.
- Proficiency in Microsoft Office and ability to travel locally and regionally as needed.
