- Manage a small patient panel of up to 225 seniors with complex needs

- MD or DO degree in Internal Medicine, Family Medicine, Geriatric Medicine, or a related specialty
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Minimum of 1 year clinical experience in geriatrics or family practice
- Leadership experience preferred
- Excellent communication, leadership, and analytical skills
- Proficiency with Microsoft Office
- Ability to travel locally and occasionally regionally as needed
