- Lead a primary care outpatient center focused on senior care and value-based medicine.
- Guide clinical teams, manage PCPs and NPs, and drive quality patient outcomes.
- Support organizational mission to deliver accessible, high-quality, and personalized care.

- Direct daily clinical operations of the primary care center.
- Oversee a small patient panel (up to 240 patients), ensuring individualized care for seniors with multiple chronic conditions.
- Supervise, mentor, and support PCPs, NPs, and other clinical staff.
- Coordinate with in-house consulting specialists, including cardiology and podiatry professionals.
- Implement and monitor value-based care initiatives to achieve superior patient outcomes.
- Lead staff training in clinical best practices, leadership skills, and operational efficiency.
- Contribute to center financial performance and participate in growth and outreach activities.

- Medical degree (MD or DO) in Internal Medicine, Family Medicine, Geriatrics, or a closely related specialty.
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Minimum of one year clinical experience in geriatric or family practice; leadership experience is preferred.
- Strong leadership and communication skills, with proficiency in Microsoft Office.
- Ability to travel locally and occasionally regionally as needed.
