Medical Director, Primary Care - Summary- Full-time physician leadership role in an outpatient primary care setting
- Lead a value-based model of care designed for medically complex senior patients
- Work with a small patient panel to facilitate high-touch, relationship-based care
- Practice in Philadelphia, PA, with strong clinical and administrative support
- Lead and manage a multidisciplinary team of Primary Care Physicians (PCPs) and Nurse Practitioners (NPs)
- Oversee clinical operations and ensure delivery of high-quality, evidence-based care
- Develop and implement care plans for a senior patient panel with multiple chronic conditions
- Promote value-based care initiatives and optimize patient outcomes
- Participate in strategic planning and continuous quality improvement efforts
- Engage in ongoing leadership training and professional development programs
- Coordinate with in-house consulting specialists and holistic wellness providers
- Facilitate patient access to transportation and supplemental services

- MD or DO degree required
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Minimum of 1 year clinical experience in a geriatric or primary care setting
- Leadership experience preferred
- Strong communication, organizational, and team management skills
- Proficiency in Microsoft Office applications
- Ability to travel locally; occasional regional travel may be required
- Experienced clinician with a passion for senior care and value-based medicine
- Demonstrated leadership ability and a collaborative approach to team management
