The Medical Director, Primary Care is responsible for overseeing a small patient panel, managing a team of primary care physicians (PCPs) and nurse practitioners (NPs), and ensuring the delivery of high-quality, holistic healthcare services. The Medical Director, Primary Care will work closely with a multidisciplinary team to provide and coordinate care for seniors with an average of 3-5 chronic conditions. Medical Director, Primary Care - Summary

Duties & Responsibilities
- Lead and manage a team of PCPs and NPs, ensuring the highest standards of patient care.
- Oversee a patient panel of 60-90 seniors, focusing on effective management of chronic conditions.
- Collaborate with in-house specialists including cardiology and podiatry, as well as holistic health providers.
- Implement and support value-based care initiatives to enhance patient outcomes and satisfaction.
- Participate in ongoing training such as PCP Black Belt Training, Medical Director and Center Operations, and Leadership Pathways Program.
- Engage with programs in medical economics, center financial performance, and growth initiatives.
- Foster a culture of collaboration, continuous improvement, and patient-centered care.
- Ensure compliance with organizational policies, procedures, and regulatory requirements.

Qualifications & Requirements
- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- At least one year of clinical experience in a geriatric or family practice setting.
- Demonstrated leadership skills, strong communication, and analytical capabilities.
- Proficiency in Microsoft Office and ability to travel locally and occasionally regionally.
