As the Medical Director, Primary Care, you will serve as a key clinical leader responsible for overseeing the delivery of value-based care to a small panel of patients, primarily seniors with multiple chronic conditions. You will manage a dedicated team of primary care providers (PCPs) and nurse practitioners (NPs), ensuring the highest standards of patient care and operational excellence. Medical Director, Primary Care – Summary- Lead the clinical operations of a primary care center dedicated to senior health.
- Oversee a small patient panel (60-90 patients) for personalized, value-based care.
- Guide and support a team of PCPs and NPs in delivering high-quality healthcare.
- Champion the mission to provide affordable, VIP-level care to seniors.
- Manage day-to-day clinic operations and ensure the delivery of exceptional patient care.
- Supervise and mentor primary care providers and nurse practitioners.
- Implement and uphold clinical protocols, policies, and best practices.
- Provide direct patient care to a limited panel of seniors with complex needs (3-5 chronic conditions).
- Participate actively in the selection, training, and ongoing development of clinical staff.
- Collaborate with in-house consulting specialists in areas such as cardiology and podiatry.
- Engage in holistic health initiatives, including acupuncture, Tai Chi, yoga, and nutrition programs for patient wellness.
- Monitor and report on clinical and financial performance metrics to ensure value-based outcomes.
- Foster a culture of continuous improvement and patient-centered care.

- Medical degree (MD or DO) with board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- At least one year of clinical experience in a geriatric or family practice setting.
- Demonstrated leadership experience preferred.
- Strong leadership, communication, interpersonal, and analytical skills.
- Proficiency in Microsoft Office Suite.
- Ability to travel locally and regionally as required.
