Medical Director, Primary Care - Summary

Duties & Responsibilities
- Directly manage a panel of 60-90 senior patients with multiple chronic conditions, ensuring high-quality, patient-centered care.
- Oversee and mentor a team of PCPs and NPs, fostering a collaborative and supportive clinical environment.
- Implement and monitor value-based care initiatives to improve patient outcomes and organizational performance.
- Participate in and lead training programs including PCP Black Belt Training, Medical Director and Center Operations, and Leadership Pathways.
- Collaborate with in-house consulting specialists in areas such as cardiology and podiatry to ensure comprehensive patient care.
- Support and integrate holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling into patient care plans.
- Ensure compliance with organizational policies, regulatory standards, and best practices in clinical care delivery.
- Utilize data analytics and medical economics to inform decision-making and drive financial and clinical performance.
- Engage in community outreach and patient engagement initiatives, including coordination of door-to-door transportation services for patients.

Qualifications & Requirements
- Medical degree (MD or DO) in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Minimum of one year of clinical experience in a geriatric or family practice setting; leadership experience preferred.
- Demonstrated ability to lead clinical teams and manage operational aspects of a healthcare center.
- Strong communication, leadership, and analytical skills.
- Proficiency in Microsoft Office and willingness to travel locally and occasionally regionally as required.
