As a Medical Director, Primary Care, you will oversee a small patient panel of seniors, typically managing between 60 and 90 patients, most of whom live with three to five chronic conditions. This approach allows for a personalized, relationship-based care model. You will be responsible for supervising primary care physicians (PCPs) and nurse practitioners (NPs), ensuring the highest standards of care are delivered. The role focuses on value-based care, promoting better health outcomes and patient satisfaction while optimizing efficiency and effectiveness within the clinic.

- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or related specialty
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Minimum of 1 year clinical experience in geriatric or family practice setting
- Demonstrated leadership experience preferred
- Strong leadership, communication, and analytical skills
- Proficiency in Microsoft Office
- Ability to travel locally and occasionally regionally
- Experienced physician with a passion for senior care
