As the Medical Director – Primary Care, you will manage a close-knit patient panel, supervise primary care providers (PCPs) and nurse practitioners (NPs), and ensure that all care is delivered in accordance with the principles of value-based medicine. The patient population consists primarily of seniors with three to five chronic diseases, requiring a holistic and compassionate approach to healthcare. The clinic supports both patients and providers with comprehensive resources, including in-house consulting specialists, holistic health services, and door-to-door transportation for patients. Medical Director – Primary Care Summary
- Lead the clinical strategy and care delivery in a primary care outpatient setting
- Supervise and mentor PCPs and NPs
- Manage a small, focused patient panel of 60-90 patients to ensure high-quality, personalized care
- Champion value-based care and improve health outcomes for senior patients
- Engage in ongoing leadership and professional development through comprehensive training programs
Duties & Responsibilities
- Oversee daily clinical operations of the outpatient center, ensuring smooth and effective healthcare delivery
- Supervise and provide guidance to PCPs and NPs, fostering a collaborative and supportive environment
- Deliver direct patient care to a panel of seniors with multiple chronic conditions
- Implement evidence-based medical practices and maintain compliance with healthcare regulations
- Collaborate with in-house specialists, including cardiology and podiatry, to coordinate patient care
- Promote holistic health services, such as acupuncture, tai chi, yoga, and nutrition counseling
- Participate in the Leadership Pathways Program and ongoing training in medical economics, center financial performance, and center operations
- Monitor and improve quality metrics, patient satisfaction, and clinical outcomes
- Foster growth and support clinic expansion efforts

Qualifications & Requirements
- Medical degree (MD or DO) in Internal Medicine, Family Medicine, Geriatrics, or a related specialty
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Minimum of one year clinical experience in a geriatric or family practice setting
- Demonstrated leadership experience preferred
- Excellent communication, leadership, and analytical skills
- Proficiency in Microsoft Office
- Ability to travel locally and occasionally regionally as needed
