As part of this mission, we are seeking a Medical Director – Primary Care to lead our clinical team and drive our commitment to value-based care and holistic health services. As Medical Director – Primary Care, you will play a critical role in overseeing day-to-day clinical operations, ensuring high-quality care delivery, and managing a dedicated team of primary care physicians (PCPs) and nurse practitioners (NPs). You will champion the organization's mission to honor and support seniors, many of whom have complex health needs, and will be instrumental in shaping the standard of care provided in our clinics. The position involves working closely with a small patient panel to build strong provider-patient relationships, maintain continuity of care, and facilitate comprehensive treatment plans for individuals with multiple chronic conditions.
Our clinics feature in-house consulting specialists in areas such as cardiology and podiatry, as well as holistic health offerings including acupuncture, tai chi, yoga, and nutrition counseling. Patients also benefit from door-to-door transportation services, ensuring equitable access to care. The Medical Director – Primary Care will have access to robust training programs, including leadership pathways, operational management, and medical economics, further enhancing their capacity to lead and innovate.

Medical Director – Primary Care Summary

Duties & Responsibilities
- Supervise and support a clinical team of PCPs and NPs, ensuring adherence to quality and compliance standards.
- Manage a small patient panel (60-90 patients), prioritizing continuity and personalized care for seniors with complex health needs.
- Implement and monitor value-based care initiatives to drive improved patient outcomes.
- Participate in organizational leadership development programs, such as PCP Black Belt Training, Medical Director training, and operational management pathways.
- Engage in center financial performance and growth initiatives, collaborating with executive and administrative teams.
- Coordinate with consulting specialists to integrate cardiology, podiatry, and holistic health services into patient care plans.
- Foster a culture of collaboration, innovation, and patient-centeredness within the clinic environment.
Salary & Benefits

- Relocation assistance and continuing education support ($3,500 + 1 week).
- Work/life balance with a Monday–Friday schedule and clinic hours from 7:30 am to 5:00 pm.
Qualifications & Requirements
- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Minimum of 1 year of clinical experience in a geriatric or family practice setting.
- Demonstrated leadership experience or a strong desire to develop leadership skills.
- Exceptional communication, analytical, and organizational skills.
- Proficiency in Microsoft Office and ability to travel locally and occasionally within the region.
