Medical Director – Primary Care Summary- Lead a multidisciplinary team in a high-performing outpatient primary care center.
- Oversee care delivery for a senior patient population with multiple chronic conditions.
- Drive initiatives that enhance value-based care and patient satisfaction.
- Participate in leadership training and professional development programs.
- Promote holistic and integrative care services for optimal patient outcomes.
- Manage a small patient panel (60-90 patients) to ensure individualized care.
- Supervise and mentor primary care providers and nurse practitioners.
- Implement and monitor value-based care models to improve quality and efficiency.
- Facilitate patient access to in-house consulting specialists (e.g., cardiology, podiatry).
- Oversee holistic health services and promote wellness initiatives among patients.
- Conduct regular team meetings and participate in leadership development programs.
- Support operational excellence and financial performance of the center.
- Ensure compliance with clinical protocols and best practices in senior care.

- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Board certification or board eligibility in FM, IM, or Geriatric Medicine.
- Minimum of 1 year clinical experience in a geriatric or primary care setting.
- Leadership experience preferred.
- Strong leadership, communication, and analytical skills.
- Proficiency in Microsoft Office and willingness to travel locally and regionally as needed.
