As the Medical Director, Primary Care, you will oversee a team of Primary Care Physicians (PCPs) and Nurse Practitioners (NPs), guiding them in the delivery of exceptional healthcare services. You will utilize your clinical expertise and leadership abilities to manage operations, drive clinical excellence, and mentor providers. Our value-based care approach focuses on seniors with multiple chronic conditions, and we invest in your ongoing professional development through specialized leadership and operational training programs. These include PCP Black Belt Training, Medical Director and Center Operations training, and participation in our Leadership Pathways Program.

Medical Director, Primary Care - Summary- Leadership role overseeing primary care delivery for seniors with chronic diseases
- Physician-led, mission-driven, value-based care organization
- Located in Jacksonville, FL (full-time)
- Focus on patient-centered, comprehensive healthcare
- Manage and mentor a team of PCPs and NPs, overseeing clinical operations
- Deliver high-quality, value-based primary care to a small patient panel (60-90 seniors)
- Focus on care for seniors with 3-5 chronic conditions
- Participate in and lead clinical and operational training programs
- Implement and monitor clinical quality standards and best practices
- Collaborate with in-house specialists in cardiology, podiatry, and more
- Promote holistic health services, including acupuncture, Tai Chi, yoga, and nutrition
- Ensure a high standard of patient satisfaction and quality care delivery
- Engage in strategic planning for clinic growth and performance

- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or related specialty
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- At least 1 year of clinical experience in geriatric or family practice settings
- Leadership experience is preferred
- Strong leadership, communication, and analytical skills
- Proficiency in Microsoft Office and willingness to travel locally/regionally as needed
- Passionate about delivering high-quality, compassionate care to seniors
- Experienced in clinical leadership with a focus on patient-centered outcomes
- Comfortable leading teams and mentoring clinical staff
