Medical Director – Primary Care Summary
- Lead and manage a multidisciplinary clinical team in an outpatient primary care environment.
- Champion the delivery of value-based care for senior patients with complex health needs, focusing on chronic disease management and holistic wellness.
- Oversee the clinical operations, ensuring patient care excellence and adherence to organizational standards.
- Promote best practices in quality, patient safety, and operational efficiency.
- Mentor and develop primary care staff, including physicians and nurse practitioners.
Duties & Responsibilities
- Manage a patient panel of 60–90 seniors, primarily with multiple chronic conditions.
- Supervise, coach, and evaluate primary care providers and nurse practitioners.
- Ensure effective implementation of value-based care models and quality improvement initiatives.
- Collaborate with in-house specialists in cardiology, podiatry, and other disciplines to support comprehensive patient care.
- Participate in leadership and clinical training programs, including PCP Black Belt Training, Medical Director Essentials, and Leadership Pathways.
- Engage in operational and financial oversight, contributing to the center’s medical economics and growth strategies.
- Foster a culture of collaboration, continuous learning, and patient advocacy.
- Utilize technology and data analytics to inform clinical decisions and drive positive health outcomes.
- Support holistic health initiatives, including acupuncture, Tai Chi, yoga, and nutritional counseling.
- Ensure patient accessibility by supporting transportation services for those in need.

Qualifications & Requirements
- Medical degree (MD or DO) in Internal Medicine, Family Medicine, Geriatrics, or a closely related specialty.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Minimum of one year of clinical experience in a geriatric or family practice setting.
- Demonstrated leadership experience in a healthcare environment is preferred.
- Strong leadership, team management, and communication skills.
- Analytical abilities to interpret clinical data and drive operational improvement.
- Proficiency in Microsoft Office Suite and ability to travel locally and occasionally regionally as required.
