As the Medical Director – Primary Care, you will oversee a small patient panel, ensuring each patient receives personalized, VIP-level attention. With a focus on seniors managing multiple chronic illnesses, you will lead a multidisciplinary team of Primary Care Physicians (PCPs) and Nurse Practitioners (NPs), driving clinical excellence and operational efficiency. You will play a pivotal role in shaping the patient experience, implementing quality initiatives, and participating in organizational leadership programs designed to enhance your skills in medical economics, center operations, and growth strategies.
The organization offers a holistic approach to health, integrating in-house consulting specialists such as cardiology and podiatry, as well as complementary services including acupuncture, tai chi, yoga, and nutrition counseling. Patients are provided with door-to-door transportation, at no charge, ensuring ease of access to care. Medical Director – Primary Care Summary- Lead a multidisciplinary team in an outpatient primary care setting focused on seniors with complex, chronic conditions
- Deliver personalized, value-based care within a physician-led, mission-driven organization

- Direct and manage clinical staff, including PCPs and NPs
- Implement value-based care initiatives for seniors with multiple chronic diseases

- Collaborate with in-house consulting specialists and wellness service providers
- Support holistic patient care through integration of wellness services and patient transportation

- MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or related specialty
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Minimum one year of clinical experience in geriatric or family practice setting
- Demonstrated leadership experience preferred
- Strong communication, leadership, and analytical skills
- Proficiency in Microsoft Office
- Ability to travel locally and regionally as needed
- Experienced in managing seniors with complex health needs
