As a Licensed Professional Counselor (LPC), you will serve as an integral part of the primary care team, working closely with medical professionals to identify, triage, and manage patients with behavioral health needs. This full-time opportunity allows you to provide vital behavioral health services to patients referred for counseling and mental health support. The role encompasses a broad range of counseling services, including but not limited to therapy for depression, anxiety, grief, trauma, anger, chronic disease, and behavioral management, as well as children and family counseling, life stage coaching, marital and couples therapy, substance abuse assessments, and mental health crisis management.
The Licensed Professional Counselor (LPC) position involves frequent collaboration with primary care providers, offering brief consultative contacts, assessments, and ongoing treatment plans. You will deliver counseling both one-on-one and in group settings, perform mental health evaluations, provide crisis response, and coordinate care with external partners. The role also requires participation in quality improvement initiatives and the integration of behavioral health services with primary care through consistent team collaboration and consultation.

Licensed Professional Counselor (LPC) - Summary- Provide comprehensive behavioral health services within an integrated primary care environment.
- Deliver patient-centered counseling and therapy to diverse patient populations, including adults, children, families, and couples.
- Support HCHC's mission of accessible, affordable healthcare for all community members.
- Assist primary care providers in identifying and treating mental health and psychosocial disorders.
- Conduct clinical assessments and formulate diagnoses and treatment plans.
- Deliver individual, group, and family counseling services.
- Perform crisis intervention, behavioral modification, and supportive counseling.
- Document mental health services and coordinate care with internal teams and external stakeholders.
- Educate patients, caregivers, and staff on mental health topics.
- Participate in quality improvement initiatives and data collection.
- Uphold patient confidentiality and privacy according to all regulations.
- Collaborate with interpreter services as needed.
- Maintain professional licensure and adhere to ethical standards.

- Current and unrestricted license as an LPC or LCSW.
- Must not be sanctioned under Medicaid or Medicare.
