The Licensed Practical Nurse (LPN) - Chronic Care Management plays a vital role in delivering quality healthcare to patients with chronic conditions. As a full-time member of the healthcare team, the LPN provides essential support and coordination for patients requiring ongoing medical management. This position is dedicated to improving patient health outcomes through careful monitoring, communication, and collaboration with other healthcare professionals.
Key Responsibilities
- Provide a minimum of 20 minutes of chronic care management services to each patient every 30-day period, ensuring comprehensive attention to ongoing health needs.
- Facilitate continuous access to chronic care management services, allowing patients to contact healthcare providers promptly to address urgent or emerging health concerns related to chronic conditions.
- Promote continuity of care by ensuring patients consistently connect with a designated practitioner or member of the care team, and assist in scheduling successive appointments for ongoing management.
- Oversee and coordinate care transitions between various healthcare providers and settings, supporting seamless changes in patient care environments.
- Work closely with home health providers and community-based clinical service providers, ensuring comprehensive and coordinated care according to each patient's needs.
- Enhance opportunities for patients and their caregivers to communicate with the healthcare team, providing support and information as needed.
- Conduct systematic and thorough assessments of each patient’s medical, functional, and psychosocial needs, using these assessments to inform ongoing care strategies.
- Implement system-based approaches to guarantee patients receive all recommended preventive care services in a timely manner.
- Perform medication reconciliation, reviewing adherence and checking for potential drug interactions, and assist patients with the self-management of their medications.
- Create and maintain a patient-centered care plan that reflects the individual choices and values of each patient, and ensure patients receive a written or electronic copy of their personalized care plan.
Qualifications
- Current and valid LPN (Licensed Practical Nurse) license required.
- Experience in chronic care management is preferred but not mandatory.
- Strong interpersonal and communication skills to effectively interact with patients, caregivers, and multidisciplinary healthcare teams.
- Demonstrated ability to work collaboratively in a team environment and coordinate care across various healthcare providers and settings.
