Licensed Practical Nurse (LPN) - Summary- Deliver professional nursing care to adult and geriatric patients in an outpatient family health clinic setting.
- Utilize nursing principles to assess, plan, and assist with treatments and procedures.
- Support the overall mission of enhancing the health and readiness of service members, veterans, and their families.
- Provide telehealth nursing services, including managing telephone messages and secure digital communications to facilitate patient care coordination.
- Perform and complete assigned duties in patient care for diverse populations, adapting to varying patient conditions.
- Apply clinical judgment in the selection and sequence of procedures and treatments based on patient assessments.
- Accurately document, report, and monitor relevant patient information.
- Identify and respond to patient needs, including medication administration, equipment-assisted care, and patient/family education.
- Administer prescribed medications (oral, topical, subcutaneous, intramuscular, intravenous) and perform treatments according to policy.
- Recognize urgent or emergent patient situations, seek assistance as needed, and initiate emergency interventions as directed.
- Assist with complex diagnostic tests and specialized procedures, including patient preparation and equipment handling.
- Orient new staff and ancillary personnel to clinic policies, procedures, and standards of practice.

- Graduation from an accredited nursing program (community college, junior college, university, or vocational).
- Current, full, active, and unrestricted Licensed Practical Nurse (LPN) license.
- Certification in Basic Life Support (BLS) from the American Red Cross (ARC) or American Heart Association (AHA).
- Minimum of 12 months hands-on LPN experience in an adult primary care outpatient setting within the last 24 months.
- U.S. Citizenship required.
- Physical ability to lift up to 45 pounds independently and heavier weights with assistance or mechanical devices.
- Demonstrates strong clinical skills and sound judgment in patient care delivery.
- Experience with telehealth and digital communication tools for patient management.
