As an LCSW, you will be an integral part of a dedicated behavioral health team that includes fellow LCSWs, a Psychiatric Nurse Practitioner, and a Psychiatrist. Licensed Clinical Social Worker (LCSW) - Summary
- Full-time, permanent position within a nonprofit healthcare system
- Integrated behavioral health and primary care setting
- Four 10-hour workdays each week, supporting work-life balance
- Average 8 to 10 patients per day, allowing for meaningful patient engagement
- Collaborative, multidisciplinary team including LCSWs, Psychiatric NP, and Psychiatrist
- 1:8 phone-only call rotation
- Epic EMR system utilized for streamlined documentation and workflow
Duties & Responsibilities
- Provide evidence-based behavioral health assessments, interventions, and treatment planning for a diverse patient population
- Deliver care in close coordination with primary care providers to support an integrated care model
- Maintain accurate and timely documentation within the Epic EMR system
- Participate actively in multidisciplinary team meetings and case conferences
- Engage in ongoing communication with patients, families, and care teams to promote patient-centered outcomes
- Contribute to the development and execution of individualized treatment plans
- Support crisis intervention and risk assessment as needed
- Provide education and resources to patients and families regarding behavioral health and wellness
- Participate in the 1:8 phone-only call rotation to provide after-hours support as needed
