As a Licensed Clinical Social Worker (LCSW), you will play a pivotal role in delivering behavioral health services, from mental health assessments and crisis interventions to ongoing counseling and life-stage coaching. Your work will help patients navigate challenges such as depression, anxiety, trauma, chronic disease, substance use, and family dynamics, while connecting them with essential community resources.
In this full-time role, you will collaborate daily with primary care providers to recognize and address mental health disorders and psychosocial concerns. The Licensed Clinical Social Worker (LCSW) facilitates "warm handoffs," ensuring patients receive seamless behavioral health support within the primary care setting. You will be responsible for conducting clinical assessments, formulating diagnoses and treatment plans, and delivering both individual and group counseling for adults, children, and families. Your contributions will extend to care coordination, crisis management, documentation, and participation in continuous quality improvement initiatives.

Licensed Clinical Social Worker (LCSW) - Summary- Non-profit, patient-centered healthcare organization
- Integrated behavioral health and primary care setting
- Direct counseling and case management for diverse populations

- Assist primary healthcare providers in identifying and treating mental health disorders and psychosocial issues
- Assess clinical status of patients through brief consultative contacts and "warm handoffs"
- Formulate initial diagnoses and develop individualized treatment plans
- Provide one-on-one and group counseling for adults, children, and families
- Deliver therapy for depression, anxiety, grief, trauma, anger management, chronic disease, substance use, and life transitions
- Conduct mental health assessments and crisis interventions
- Monitor and support clients with persistent mental illness
- Offer behavioral modification and supportive counseling
- Educate patients, caregivers, and staff on mental health topics
- Document clinical services and coordinate care with external stakeholders
- Refer patients for specialty care as appropriate
- Contribute to quality improvement and data collection initiatives
- Protect patient privacy and confidentiality; comply with relevant regulations
- Collaborate with interpreter services as needed
- Participate in team meetings and continuous performance improvement

- Current and unrestricted license as an LCSW or LPC (preference for LCSW)
- Eligibility to practice without sanctions under Medicaid or Medicare
- Compliance with all federal, state, and professional licensing requirements
- Prior experience in a community health center preferred, but not required
- Experience with integrated care models is desirable
- Ability to provide both short-term solution-focused and longer-term therapy
- Experience with limited-resource populations is a plus
- Bilingual ability in Spanish is desirable, not required
- Strong interpersonal, organizational, and communication skills
- Comfort with electronic health records and telehealth platforms
- Demonstrates cultural competence and sensitivity to diverse backgrounds
- Comfortable working with both adults and children
- Effective at collaborating within multidisciplinary teams
- Adaptable, compassionate, and committed to equitable care
- Capable of handling crisis situations calmly and professionally
- Physical requirements include light work (lifting up to 10 pounds), regular office mobility, and prolonged sitting
