Licensed Clinical Social Worker (LCSW) - Summary
The Licensed Clinical Social Worker (LCSW) will serve as a Behavioral Health Consultant (BHC) working within a primary care setting. This role is designed for skilled professionals who can provide integrated behavioral health services, including brief, evidence-based assessment and intervention, all performed under the Primary Care Behavioral Health (PCBH) model. Success in this position requires completion of mandatory training phases, close collaboration with healthcare teams, and management of a full patient caseload. The LCSW will help drive population health initiatives while maintaining clinical quality standards in a dynamic, patient-centered environment.
Duties & Responsibilities
- Provide behavioral health consultation in a primary care environment, adhering to the PCBH model and DHA Procedural Instructions.
- Complete and pass Phase I (face-to-face) and Phase II (core competency) DHA BHC training within the specified timeframe to ensure readiness for independent practice.
- Deliver brief, evidence-based assessments and interventions for various behavioral health challenges, such as depression, anxiety, substance misuse, stress, insomnia, pain, obesity, diabetes, tobacco use, and other behaviors impacting overall health.
- Utilize population health approaches, including brief (15- to 30-minute) appointments, brief episodes of care, same-day service availability, and leading educational classes.
- Engage with primary care managers to maximize referrals and educate staff members on appropriate referral pathways.
- Provide group education on relevant topics (e.g., stress management, sleep enhancement, chronic pain), formatted for general primary care settings.
- Communicate effectively with patients, families, and both medical and nonmedical staff, demonstrating professionalism even in challenging situations.
- Document care and recommendations in the electronic medical record in accordance with established standards and timelines.
- Collaborate with primary care managers for timely feedback and support stepped-care services for a variety of patient needs.
- Participate in team initiatives to develop and refine clinical pathways and service delivery programs within the clinic.
- Supervise and mentor behavioral health providers in training or not yet fully licensed, as required.
- Contribute to research and quality improvement projects related to primary care behavioral health services.
- Attend required meetings, trainings, and conferences to support ongoing professional development and improve clinical care quality.
- Support clinical initiatives such as pain and sleep pathways and participate in peer review processes as needed.

Qualifications & Requirements
- Master of Social Work (MSW) degree from a CSWE-accredited institution.
- Current, unrestricted license to independently practice clinical social work in one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
- At least one year of experience as a licensed clinical social worker, psychologist, or licensed professional counselor.
- One year of recent experience (within the last three years) in evaluating and treating patients with pain, psychological factors affecting physical health, or complex medical issues.
- Experience working as a behavioral health provider in a Patient-Centered Medical Home (PCMH) or similar integrated care setting preferred.
- At least 12 months of experience in the last 24 months using computers, basic word processing, data entry, and electronic medical record systems.
- Proficiency in motivational interviewing and a solid understanding of population health approaches are desirable.
- U.S. Citizenship is required.
