Duties & Responsibilities
- Manage complex symptom control, including both pain and non-pain symptoms, for patients with serious or life-limiting illnesses
- Conduct inpatient and outpatient palliative care consultations, providing holistic and individualized care plans
- Coordinate end-of-life care planning with patients, families, and multidisciplinary care teams to ensure understanding and support
- Collaborate with physicians, advanced practice providers (APPs), nurses, social workers, chaplains, and community partners to deliver comprehensive care
- Mentor, educate, and support fellows, residents, medical students, and other clinical staff
- Engage in quality improvement and clinical program development initiatives, contributing to the advancement of hospice and palliative care services
- Participate in home hospice visits and utilize telehealth technology to reach and support patients in various settings
- Participate in the on-call rotation, including weekend home call every 6-8 weeks, ensuring continuity of care

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Board Eligible or Board Certified in Hospice and Palliative Medicine (HPM)
- Active, unrestricted U.S. medical license
- Exceptional communication skills and commitment to interdisciplinary collaboration
- Experience in medical education, mentorship, or clinical program development is valued
