Home Health Therapy Manager - Summary
- Supervises and mentors a dynamic clinical therapy team—PTs, PTAs, OTs, COTAs, and SLPs—in a home health environment.

Duties & Responsibilities
- Provide day-to-day supervision, oversight, and mentorship to the clinical therapy team.
- Coordinate and monitor individualized therapeutic plans of care for patients.
- Ensure safe, cost-effective, and evidence-based delivery of rehabilitative services in the home setting.
- Lead staff engagement initiatives, facilitate ongoing communication, and deliver training sessions to maintain high-quality standards.
- Oversee compliance with state licensure requirements, organizational policies, and regulatory standards.
- Manage patient care episodes, including changes to plans of care, clinical evaluations, and recommendations for continued therapy or discharge.

Qualifications & Requirements
- Bachelor’s degree in a therapy discipline—physical therapy, occupational therapy, or speech-language pathology.
- At least 2 years of experience in home care as a PT, OT, or SLP.
- Minimum of 1 year of supervisory experience in a related healthcare setting.
- Current and valid state licensure in the respective therapy discipline.
- Ability to regularly lift, push, pull, and support up to 50 pounds, including patient positioning and equipment movement.
- Demonstrated expertise in managing patient care episodes, modifying plans of care, and performing advanced clinical evaluations.
- Valid driver’s license, current insurance, and reliable transportation are required to perform home visits and related duties.
