Physical Therapist (PT) – Home Health Summary

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license in Illinois.
- At least one year of experience in a healthcare facility, preferably with home health exposure.
- Ability and willingness to travel within the designated coverage area (up to 50%).
- Valid driver’s license, reliable vehicle, and current auto insurance.
- Physical capacity to lift, push, pull, and support up to 50 pounds as necessary for patient care.
