As a Home Health Physical Therapist (PT), you will play a critical role in our interdisciplinary care team, working closely with patients, their families, and other healthcare professionals to evaluate, plan, and implement effective physical therapy interventions. Home Health Physical Therapist (PT) - Summary
- Provide direct physical therapy services to patients in their homes, ensuring high standards of care and positive clinical outcomes.

Duties & Responsibilities
- Deliver comprehensive physical therapy services in accordance with established standards of practice and regulatory guidelines.
- Evaluate patients' functional status, mobility, strength, and ability to perform daily activities.
- Develop and implement evidence-based treatment plans tailored to each patient's needs.
- Monitor patient progress, adjust therapy interventions as needed, and document all care provided.
- Educate patients and their families on techniques to improve mobility, prevent injury, and manage chronic conditions at home.
- Work collaboratively with nurses, physicians, occupational therapists, speech therapists, and other team members to optimize care outcomes.
- Direct and supervise support staff to assist in achieving patient goals.
- Serve as a resource and provide leadership to fellow staff members in the delivery of physical therapy services.
- Participate in quality improvement initiatives and community outreach efforts to promote awareness of home health physical therapy.
- Maintain accurate and timely documentation of assessments, treatment goals, care plans, and progress notes.
- Ensure all therapy practices comply with organizational policies and relevant legal, ethical, and professional standards.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state in which services are provided.
- Minimum of one (1) year of experience in a healthcare facility, preferably in home health or a related setting.
- Demonstrated ability to travel within the assigned branch or office coverage area, up to 50% of the time as needed.
- Possession of a dependable vehicle, valid driver’s license, and up-to-date auto insurance.
- Strong clinical assessment, organizational, and communication skills.
- Comfortable working in a physically demanding environment, including handling, lifting, and transferring patients when necessary.
