The Home Health Physical Therapist (PT) provides direct patient care, evaluates functional abilities and outcomes, and develops individualized plans of care. Duties & Responsibilities
- Promote the organization’s philosophy, mission, and administrative policies to ensure quality patient care in all activities.
- Provide direct, hands-on physical therapy to patients in their homes, including comprehensive evaluations and ongoing assessments of functional needs and progress.
- Collaborate with physicians, other healthcare professionals, and family members to design, implement, and adjust individualized patient care plans.
- Develop therapy goals and interventions that are measurable, attainable, and tailored to each patient’s unique needs and circumstances.
- Direct and supervise the work of support staff to facilitate patient progress toward established functional goals.
- Document evaluations, care plans, and treatment progress in accordance with professional standards and organizational policy, ensuring that all records are accurate, timely, and compliant with regulatory requirements.
- Maintain up-to-date prescriptions and required signatures as part of the patient care process.
- Serve as a positive representative of physical therapy in the community by engaging in outreach and education as appropriate.

Qualifications & Requirements
- Graduate of an accredited physical therapy program approved by the American Physical Therapy Association (APTA).
- Current, unrestricted Physical Therapist (PT) license valid in the state of practice.
- Minimum of one year of experience providing direct patient care in a healthcare facility, with preference for home health or community-based experience.
- Ability and willingness to travel within the designated branch or office coverage area, as patient care demands (up to 50% travel).
- Reliable transportation, a valid driver’s license, and current auto insurance in compliance with state regulations.
- Physical capacity to lift 50-100 lbs and perform tasks in a physically demanding home care environment.
