Home Health Physical Therapist (PT) - Summary
- Deliver direct, in-home physical therapy services to patients recovering from illness, injury, or surgery.
- Promote the organization's philosophy and mission by ensuring high standards of care and patient satisfaction.
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes.

Duties & Responsibilities
- Assess patients' functional abilities and needs through comprehensive evaluations conducted in-home.
- Develop, implement, and regularly update individualized care plans tailored to each patient, involving significant others when appropriate.
- Deliver direct physical therapy interventions designed to improve mobility, strength, balance, and independence.
- Document all patient evaluations, treatment plans, progress notes, and required signatures in a timely, accurate manner.
- Direct and supervise support staff to help patients achieve functional goals.
- Consult with other healthcare professionals and specialists to coordinate care and ensure holistic treatment approaches.
- Promote physical therapy awareness and education in the community, acting as a resource for staff and families.
- Adhere to all relevant regulatory, administrative, and professional standards of practice.
- Perform job functions in a physically demanding environment, including frequent lifting, transferring, and positioning of patients and equipment.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- Minimum of one year of experience in a healthcare facility or similar clinical setting.
- Ability and willingness to travel within the assigned coverage area as required to meet patient needs.
- Reliable transportation, a valid driver's license, and current auto insurance in compliance with state regulations.
- Physical ability to regularly lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and managing equipment.
