Responsibilities:
- Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
- Design and implement a plan of care for patients based on assessments, incorporating participation by family or significant others as appropriate.
- Conduct therapy practice within defined standards of care, documenting evaluations, treatment goals, and plans.
- Direct and lead support staff to achieve patients functional goals.
- Promote Elara Carings philosophy, mission, and policies to ensure high quality care.
- Provide direction and leadership, promoting physical therapy within the community.
Qualifications:
- Graduate of an American Physical Therapy Association (APTA) accredited Physical Therapy program.
- Current, unrestricted Physical Therapist (PT) license in the state of employment.
- Minimum one (1) year of experience in a healthcare facility.
- Valid drivers license, dependable vehicle, and current auto insurance per state laws.
- Willingness to travel up to 50% within branch/office coverage area.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including transferring patients and moving equipment.
