Home Health Nurse Practitioner (NP) - Summary
- Deliver patient-centered, in-home healthcare services to adult and senior populations
- Emphasize prevention, chronic disease management, and quality of life improvements
- Collaborate with a multidisciplinary care team to create comprehensive care plans
Duties & Responsibilities
- Conduct thorough health assessments and preventive care visits according to evidence-based clinical guidelines
- Manage acute and chronic conditions with development and implementation of individualized treatment plans
- Coordinate care and communicate effectively with physicians and other healthcare professionals
- Order, interpret, and evaluate diagnostic tests to inform clinical decisions
- Prescribe medications and therapies within the Nurse Practitioner scope of practice and state regulations
- Educate and empower patients and their families on health management and wellness strategies
- Document all patient interactions, assessments, and care plans accurately and in a timely manner
- Act as a resource for patients, caregivers, and the interdisciplinary care team

Qualifications & Requirements
- Master's degree in Nursing (MSN) from an accredited program
- Current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licenses in California
- Minimum of one year of experience as a Nurse Practitioner
- DEA license preferred
- Dedication to patient-centered, value-based healthcare delivery
- Excellent communication, collaboration, and organizational skills
- Ability to work independently in patients' homes and adapt to various environments
