Home Health Nurse Practitioner (NP) - Summary- Deliver in-home primary care and preventive services to a diverse, homebound patient population in Downey, CA and surrounding areas.
- Work as part of a value-based care model focused on prevention, chronic disease management, and patient empowerment.

- Conduct thorough health assessments and preventive care visits for adult and senior patients in their homes, ensuring adherence to established clinical guidelines.
- Diagnose, manage, and treat acute and chronic conditions utilizing evidence-based care plans tailored to each patient’s needs.
- Collaborate closely with physicians, interdisciplinary care teams, and other healthcare professionals to create and implement coordinated care plans.
- Order, interpret, and evaluate diagnostic tests to inform clinical decisions and optimize patient outcomes.
- Prescribe medications, therapies, and interventions in accordance with state and federal regulations, as well as scope of practice.
- Educate and empower patients and families to proactively manage health conditions, understand medications, and engage in preventive care.
- Maintain accurate and timely clinical documentation in compliance with organizational policies and regulatory guidelines.
- Participate in ongoing professional development, quality improvement initiatives, and team meetings.
- Competitive base salary ranging from $135,000 to $152,000, commensurate with experience.
- Eligibility for an annual performance bonus program.

- Master’s degree in Nursing (MSN) from an accredited program.
- Current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licenses in California.
- Minimum of one year of experience working as a Nurse Practitioner, preferably in a home health or value-based care setting.
- DEA license preferred.
- Demonstrated commitment to patient-centered, compassionate care and value-based healthcare delivery.
- Strong clinical judgment and ability to work autonomously in patient homes.
- Excellent interpersonal, communication, and organizational skills.
