Home Health Nurse Practitioner (NP) – Summary- Full-time opportunity focused on providing in-home care to adult and senior patients

- Order, interpret, and evaluate diagnostic tests to guide healthcare decisions
- Prescribe medications and therapies within scope of practice and regulatory guidelines
- Educate and empower patients to proactively manage their health and chronic conditions
- Collaborate with physicians, nurses, and other care team members to coordinate high-quality, patient-centered care

- Master’s degree in Nursing (MSN) required
- Active and unrestricted RN and Nurse Practitioner (NP) licenses in California
- Minimum of one year of experience as a Nurse Practitioner
- DEA license preferred
