- Collaborate with Advanced Practice Providers for referrals, patient follow-up, and care coordination.
- Utilize the Epic EMR for documentation and care management.
- Participate in a Monday-Thursday schedule with some on-call responsibilities.
- Maintain high standards of patient care and adhere to clinical protocols.
