Family Nurse Practitioner (FNP) / Physician Assistant (PA-C) - Summary- Provide primary care services in a family medicine setting as a Family Nurse Practitioner (FNP) or Physician Assistant (PA-C).
- Collaborate with physicians and healthcare team members to deliver coordinated, patient-focused care.
- Support the mission of Palm Health Resources by promoting patient wellness and preventive health strategies.
- Conduct comprehensive routine physical assessments using evidence-based practices and systematic clinical skills.
- Order, interpret, and evaluate diagnostic tests to accurately identify and assess patient health concerns.
- Document physical findings and develop individualized treatment plans and prognoses according to patient conditions.
- Perform physical examinations and administer preventive health measures as guided by physician protocols.
- Uphold patient rights, including privacy, confidentiality, informed consent, and the right to refuse care.
- Educate patients and families to promote self-care, health literacy, and independence.
- Adhere to all organizational policies, clinical guidelines, and departmental procedures.
- Maintain accurate, up-to-date patient records in compliance with regulatory standards.
- Participate in continuous improvement initiatives to enhance patient care outcomes.

- Graduation from an accredited Master of Science in Nursing (MSN) or Physician Assistant (PA) program.
- Current and valid Indiana RN license required.
- Active APRN or PA/CSR licensure, or eligibility to obtain necessary state licensure.
- Board certification through AANP, ANCC, or NCCPA for PA-C.
- Valid driver’s license.
- Current BLS (Basic Life Support) certification.
- Commitment to upholding clinical standards and continuous professional development.
- Demonstrates a patient-focused approach and strong interpersonal communication skills.
- Proven ability to work effectively within multidisciplinary teams.
- Experience in family medicine, primary care, or similar clinical settings.

- Full-time schedule, Monday through Friday, with typical hours from 8:00 a.m. to 5:00 p.m., supporting work-life balance.
