Family Nurse Practitioner (FNP) / Physician Assistant (PA) - Summary- Full-time position providing primary care to adult and geriatric patients
- Outpatient practice setting with a focus on comprehensive, personalized care

- Conduct comprehensive health assessments and physical examinations for adult and geriatric patients
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized treatment plans based on evidence-based guidelines
- Order, interpret, and evaluate diagnostic tests and lab results
- Prescribe medications and monitor patient responses
- Educate patients and families on health promotion, disease prevention, and chronic disease management
- Coordinate patient care with physicians, specialists, and other healthcare providers
- Maintain accurate and timely documentation in the electronic health record system
- Participate in quality improvement initiatives and team meetings
- Optional participation in on-call coverage (not required)
