Family Nurse Practitioner (FNP) - Full-Time Primary Care Role
We are seeking a dedicated and experienced Family Nurse Practitioner (FNP) to join a collaborative, patient-focused primary care practice. As an FNP, you will play a vital role in delivering comprehensive primary care services to patients across all age groups, from newborns to seniors. This full-time position offers the opportunity to work alongside a multidisciplinary team of healthcare professionals, including physicians, physician assistants, social workers, medical assistants, clerical staff, and care management teams, all committed to providing high-quality, accessible healthcare to the community.
As an integral member of the care team, the Family Nurse Practitioner (FNP) will provide preventive care, manage acute and chronic medical conditions, and perform minor office-based procedures. The FNP will have access to robust support resources, including clerical staff to assist with administrative tasks, as well as an on-site behavioral health specialist to address patients' mental and social health needs. This collaborative approach enables clinicians to focus more effectively on direct patient care and clinical decision-making.
Our practice is committed to improving healthcare accessibility for vulnerable and underserved populations. Flexible scheduling options, such as early morning, late evening, walk-in, Saturday morning, and telemedicine appointments, are available to accommodate diverse patient needs. The Family Nurse Practitioner (FNP) may also be asked to provide care for patients from other clinicians’ panels as needed, and, depending on experience and interest, may occasionally deliver primary care to home-bound patients.
Clinicians in this role are empowered to manage their own patient panels while working closely with the healthcare team to ensure comprehensive and coordinated care. - Current state-controlled substance license
