Family Nurse Practitioner (FNP) - Summary

Duties & Responsibilities
- Conduct routine wellness and preventive care visits for pediatric, adult, and geriatric patients
- Evaluate and manage acute illnesses and minor urgent healthcare needs
- Diagnose and treat chronic conditions, ensuring continuity of care
- Collaborate with physicians and other healthcare professionals to develop and implement individualized patient care plans
- Maintain accurate, detailed patient records utilizing the EPIC electronic medical record (EMR) system
- Provide patient education and counseling on health promotion, disease prevention, and lifestyle modification
- Order and interpret diagnostic tests as appropriate
- Participate in quality improvement initiatives and clinic meetings as needed
- Adhere to clinical protocols, regulatory requirements, and ethical standards

Qualifications & Requirements
- Active and unrestricted Family Nurse Practitioner (FNP) license
- Current DOT certification preferred, or willingness to obtain upon hire
- Strong clinical skills in acute, preventive, and chronic care management
- Proficiency in documentation and use of EMR systems, preferably EPIC
- Excellent interpersonal and communication skills
- Demonstrated ability to work effectively as part of a collaborative care team
- Minimum 1-2 years of experience in a primary care or outpatient setting preferred
