Family Nurse Practitioner (FNP) Position Overview
We are seeking a dedicated Family Nurse Practitioner (FNP) to join our dynamic care team in Delaware. This full-time employment opportunity focuses on delivering comprehensive primary care services in a 100% outpatient office setting. The Family Nurse Practitioner will collaborate closely with an experienced clinical team, providing patient-centered care for adults in a supportive and professionally enriching environment.
Key Responsibilities
- Deliver high-quality primary care services to patients aged 18 and older in an outpatient setting.
- Collaborate effectively with physicians, nurses, and other members of the healthcare team to ensure coordinated, holistic patient care.
- Conduct thorough patient assessments, develop treatment plans, prescribe medications, and monitor patient progress using evidence-based practices.
- Utilize the electronic medical record (EMR) system (STI, acquired by Azalea Health) to document patient encounters and maintain accurate, up-to-date records.
- Manage a patient panel of approximately 20-25 patients per day, ensuring efficient and compassionate care delivery.
- Participate in administrative duties as scheduled, with Fridays designated for administrative tasks.
- Advocate for patient needs, promote wellness, and provide education regarding preventive care and chronic disease management.
Qualifications
- Current Board Certification as a Family Nurse Practitioner (FNP).
- Licensed or eligible for licensure as a Nurse Practitioner in Delaware.
- Current or eligible Delaware DEA registration.
- Demonstrated commitment to patient-centered care and the ability to work effectively as part of a collaborative care team.
- Open to both new graduates and experienced practitioners.
