As a Family Nurse Practitioner (FNP), you will manage a diverse patient panel aged 18 and above, focusing on preventative care, diagnosis, treatment, and patient education. Family Nurse Practitioner (FNP) - Summary- Full-time Family Nurse Practitioner providing outpatient primary care services
- Collaborative and supportive clinical environment
- Focus on adult patient population (ages 18+)
- Monday through Friday schedule with dedicated administrative time
- Conduct comprehensive patient assessments, develop care plans, and deliver evidence-based primary care
- Manage acute and chronic medical conditions independently and in collaboration with the clinical team
- Order and interpret diagnostic tests as appropriate
- Prescribe medications, renew prescriptions, and monitor treatment efficacy
- Educate patients on preventive care, health maintenance, and disease management
- Maintain accurate and timely documentation in the EMR (STI by Azeala Health)
- Coordinate care with specialists, nurses, and other healthcare professionals
- Participate in quality improvement initiatives and team meetings
- Ensure compliance with clinical protocols, regulations, and organizational policies
