Family Nurse Practitioner (FNP) Job Description
Family Medicine at Greenhill is seeking a dedicated Family Nurse Practitioner (FNP) to join our primary care team in Wilmington, Delaware. This is a full-time opportunity for an experienced clinician committed to providing comprehensive, patient-centered care to individuals and families across the lifespan. The FNP will play a pivotal role in delivering preventive health services, managing acute and chronic conditions, and supporting the overall well-being of a diverse patient population.
As a Family Nurse Practitioner at our Wilmington practice, you will be an integral part of a collaborative healthcare team including physicians, physician assistants, social workers, nurses, medical assistants, clerical staff, and scribes. We are committed to minimizing administrative burdens so you can focus on direct patient care. Our practice structure provides robust support via chronic care management teams, clerical staff handling preauthorization and forms, and optional scribe support for efficient documentation and EHR management. This enables clinicians to maximize the time spent diagnosing, treating, and building relationships with patients.
Responsibilities
- Deliver comprehensive office-based primary care services to a panel of patients of all ages, from newborns through seniors.
- Provide preventive health screenings, acute illness care, chronic disease management, and select office-based procedures within your scope of practice.
- See patients from other clinicians’ panels as needed to ensure timely access to care.
- Participate in a flexible schedule, including some early morning, evening, and occasional Saturday morning shifts to accommodate patient needs.
- Optionally provide primary care services to homebound patients, based on practice needs and your experience or interest.
- Work collaboratively with a multidisciplinary team, including behavioral health specialists, to address both medical and psychosocial aspects of patient care.
- Utilize electronic health record (EHR) systems efficiently for documentation and care coordination, supported by scribe assistance as available.
Qualifications
- Graduation from an accredited Nurse Practitioner program with Family Nurse Practitioner (FNP) certification.
- Current licensure as a Nurse Practitioner in the State of Delaware.
- Unencumbered Delaware controlled substance license and DEA registration.
- Minimum of two years’ experience in a primary care or similar clinical setting.
- Pediatric experience required; home visit experience is a plus but not mandatory.
- Proficiency in using electronic health record (EHR) systems.
- Bilingual language skills are an asset but not required.
- Excellent written and interpersonal communication abilities.
- Demonstrated ability to work effectively as part of a collaborative healthcare team.
