Family Nurse Practitioner (FNP) Job OverviewThe Family Nurse Practitioner (FNP) is a vital member of the healthcare team at Family Medical Associates, P.C. This full-time role is designed for compassionate and skilled nurse practitioners dedicated to providing high-quality primary care services. As an FNP, you will conduct office visits and support a diverse patient population, collaborating closely with onsite physicians to ensure excellent continuity of care. This position is based in Massachusetts, with opportunities to work within the Canton and Seekonk communities. Key Responsibilities- Perform comprehensive in-person office visits for patients in a primary care setting, addressing a wide range of acute and chronic health concerns.
- Conduct thorough patient assessments, including health histories, physical examinations, and diagnostic evaluations, to develop and implement effective care plans.
- Collaborate with onsite physicians and interdisciplinary team members to manage chronic diseases, deliver urgent care, and provide medication management services.
- Order and interpret diagnostic tests, prescribe medications, and monitor patient progress, ensuring close follow-up and continuity of care.
- Educate patients and their families about health maintenance, disease prevention, and treatment plans to promote optimal wellness and self-care.
- Document all patient interactions and care activities in compliance with regulatory, legal, and organizational standards.
- Respond promptly to acute medical issues and facilitate referrals to specialists or additional services as clinically indicated.
- Maintain up-to-date knowledge of evidence-based practices in family medicine and primary care, participating in continuing education and quality improvement initiatives as required.
- Current licensure as a Nurse Practitioner (NP) in the state of Massachusetts.
- Certification as a Family Nurse Practitioner (FNP) from an accredited organization such as the ANCC or AANP.
- Experience in outpatient primary care is preferred, but newly certified FNPs are encouraged to apply.
- Strong clinical assessment, communication, and interpersonal skills with a commitment to providing patient-centered care.
- Ability to work effectively both independently and as part of a collaborative healthcare team.
- Proficient with electronic health records and clinical documentation systems.
