The Family Nurse Practitioner (FNP) is a vital member of the healthcare team at Family Medical Associates, P.C. Key Responsibilities
- Perform comprehensive in-person office visits for patients in a primary care setting, addressing a wide range of acute and chronic health concerns.
- Conduct thorough patient assessments, including health histories, physical examinations, and diagnostic evaluations, to develop and implement effective care plans.
- Collaborate with onsite physicians and interdisciplinary team members to manage chronic diseases, deliver urgent care, and provide medication management services.
- Order and interpret diagnostic tests, prescribe medications, and monitor patient progress, ensuring close follow-up and continuity of care.
- Educate patients and their families about health maintenance, disease prevention, and treatment plans to promote optimal wellness and self-care.
- Document all patient interactions and care activities in compliance with regulatory, legal, and organizational standards.
- Respond promptly to acute medical issues and facilitate referrals to specialists or additional services as clinically indicated.
- Maintain up-to-date knowledge of evidence-based practices in family medicine and primary care, participating in continuing education and quality improvement initiatives as required.
Qualifications
- Current licensure as a Nurse Practitioner (NP) in the state of Massachusetts.
- Certification as a Family Nurse Practitioner (FNP) from an accredited organization such as the ANCC or AANP.
- Experience in outpatient primary care is preferred, but newly certified FNPs are encouraged to apply.
- Strong clinical assessment, communication, and interpersonal skills with a commitment to providing patient-centered care.
- Ability to work effectively both independently and as part of a collaborative healthcare team.
- Proficient with electronic health records and clinical documentation systems.
