Family Medicine Physician (FM) / Internal Medicine Physician (IM) - Summary- Provide comprehensive primary care to adult patients within a multidisciplinary FQHC setting.
- Collaborate with pediatric, women’s health, behavioral health, dental, and pharmacy departments for integrated care delivery.
- Leverage advanced resources in a supportive, accredited clinical environment.

- Conduct thorough patient assessments, develop treatment plans, and deliver evidence-based primary care for adult patients.
- Diagnose and manage acute and chronic medical conditions, coordinating care with other specialties as needed.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
- Participate in quality improvement initiatives and contribute to the organization’s accreditation standards.
- Engage in limited on-call responsibilities as part of a balanced work schedule.
- Educate patients and families on preventive care, wellness, and management of chronic diseases.
- Collaborate with interdisciplinary teams, including behavioral health and pharmacy, to optimize patient outcomes.

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM).
- Unrestricted medical license (or eligibility) to practice medicine.
- Commitment to providing high-quality, patient-centered care in a community health setting.
- Strong communication, interpersonal, and organizational skills.
- Experience working in a multidisciplinary team setting is preferred.
- Demonstrated ability to manage both acute and chronic adult medical conditions.
