Duties & Responsibilities
- Provide comprehensive primary care to patients of all ages in an outpatient setting
- Conduct thorough patient assessments, develop individualized treatment plans, and manage acute and chronic conditions
- Collaborate with a team of health care professionals, including specialists and support staff, to ensure optimal patient outcomes
- Maintain accurate and timely documentation using electronic health records (EHR)
- Participate in quality improvement initiatives and adhere to evidence-based care standards
- Engage in patient education, preventive care, and health promotion activities
- Allocate 36 hours per week to direct patient care and 4 hours to administrative duties
