Family Medicine Nurse Practitioner/Physician Assistant (NP/PA) - Summary
- Join an established, patient-focused group practice with a collaborative work environment.
- Deliver high-quality primary care services to a broad patient population, including children, adults, and seniors.
- Build and manage your own patient panel while working closely with other healthcare providers.
Duties & Responsibilities
- Conduct comprehensive patient histories, physical examinations, and health assessments.
- Manage medication therapies and chronic disease management plans.
- Perform new patient consults and ongoing follow-ups for acute and chronic conditions.
- Provide care to patients across the lifespan, from pediatrics to geriatrics.
- Document all patient encounters and care plans in the electronic health record (Epic experience preferred).
- Educate patients and families on disease prevention, wellness, and treatment plans.
- Collaborate with physicians, specialists, nurses, and other members of the healthcare team.
- Maintain compliance with clinical protocols and regulatory standards.
- Demonstrate the ability to work with increasing autonomy as your patient panel develops.

Qualifications & Requirements
- Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA) in Family Medicine.
- Active Michigan license preferred or eligibility to obtain one.
- Experience in primary care or family medicine is highly valued.
- Proficiency with electronic health records; Epic experience is a plus.
- Ability to provide care to patients of all ages.
